I recently bought a Mamiya m645 with a 220 insert and my first roll came back unexposed?/?
I have researched and I realized that you need to adjust the 220 insert so it will run the thicker 120 film through it so my question is is it possible the shutter stuck on every frame because the film was pushing up against it? because I know the film made it to the ending spool and I am pretty confident my shutter fires correctly when no film is loaded.
at first that was what I figured but I know I put the film on the right way because on the finished spool I could see the back of the paper which seemed odd to me but I am pretty sure thats a sure sign that the film had to be on the right way
you would have noticed if the shutter wasn’t working, it’s a pretty distinctive noise.
I had the same problem with my yashica rangefinder, my first roll, unexposed, gutted as i did a few wedding shots on it

anyhoo I have no answer for you, faulty film? unlikely but possible.
maybe, this will sound uber stupid, maybe you put the film the wrong way around! there is a paper film back to the light sensitive material. maybe you inserted it wrong.
maybe the camera has something not quite right, something in the way.
